The Lutheran Foundation: Building a Healthier Community grant process change

Lutheran logoThe Lutheran Foundation’s Building A Healthier Community (BHC) grant process is changing as follows:

  • Grant Workshops will be offered in November 2017.
  • If you intend to apply in the next BHC grant cycle, you must submit a Concept Paper.
  • Concept Paper deadline is December 15, 2017 (noon).
    • A Committee reviews and determines if the organization should proceed with submitting an application.
  • Foundation staff will send an email to the Grant Contact with notification of the Committee’s decision.
    • If you are notified to proceed, the link to proceed with the application will be available through the Grant Portal.
  • Grant Application deadline is March 1, 2018 (noon).
  • Decisions will be made by June 30, 2018.

Updated Community Health Resource Directory from St. Joseph Community Health Foundation: Now available in Spanish

The Directory makes it easier to connect uninsured, under-insured and low-income individuals with quality healthcare resources in our community.

Allen County is blessed with many healthcare providers who want to provide quality care at a low-cost and/or for free. Finding these providers is often difficult.

2013 Community Health Resource Directory from St. Joseph Community Health Foundation

The St. Joseph Community Health Foundation has published a free community health resource directory connecting thousands of uninsured, under-insured and low income Allen County residents with healthcare resources in our community since 2004.

They have just published their sixth edition, an updated 2013 Community Health Resource Directory – and it is available online.

An on-line virtual version of this Community Health Resource Directory is now available on the St. Joseph Community Health Foundation website at www.sjchf.org or by clicking on the round icon above.

The online version includes everything in the paper directory plus:

  • search functions
  • linkage to local healthcare providers’ websites
  • maps and directions
  • healthcare hints
  • news and events listings on free and low-cost health fairs
  • personal alerts on upcoming screening events and educational programs
  • more to help anyone connect with lower cost, quality healthcare
