New County Health Rankings Resource

The Robert Wood Johnson Foundation, in collaboration with University of Wisconsin Population Health Institute released findings last week on a county by county basis. New County Health Rankings assess factors including physical environment, health behaviors, clinical care support, and social and economic factors.

How do Health Behaviors, Physical Environments, Clinical Care and Social and Economic factors contribute to Health Outcomes as determined in this project? Consider the project flow chart showing how they tie it together:
